We are an Early Intervention agency located in Brooklyn, serving children with developmental delays and their families. We are currently seeking a pleasant, detail-oriented individual to take on the role of Early Intervention case manager.

As a case manager coordinating and managing cases to ensure that children and their families receive the appropriate early intervention services, you will have the opportunity to work with service coordinators, therapy providers, and professional staff. A key skill for this position is strong interpersonal skills, being detail-oriented, and the ability to multitask.

Responsibilities include:

Serve as a liaison between therapists, service coordinators and the New York City Early Intervention Program.
Ensure that therapists are providing services as authorized in the child's service plan.
Maintaining a friendly relationship with all referral sources.
Maintaining a close relationship with all therapy providers and clinical supervisors.
Heavy email and phone contact with therapists as well as office staff.
Rapid response to office personnel as well as providers.
Following up on paperwork and phone contacts.
Working closely with other departments.
Ensure that services are provided as authorized and in a timely manner.
Monitor children's case files to ensure that all paperwork (prescriptions, therapy plans, etc.) are current.
Act as and as a liaison between agency therapists and staff from outside agencies.
Maintain proper documentation of all contact made with therapists, service coordinators, Early Intervention officials, office staff, as well as any activities and information related to a child's case.
Enter and update all case information in our agency's software program.
Ensure that all child case files meet and comply with the requirements and standards of the Early Intervention Program.

Position requirements:

Must have a strong verbal and written command of the English Language
Bachelor's degree in a social or human services field (i.e. psychology, sociology, social work, etc.)
Friendly, caring, outgoing personality
Must be extremely meticulous and pay close attention to details
Strong interpersonal skills
Ability to multitask
Computer literate
Experience working in a high volume work environment where organizational and multi-tasking skills are essential

Salary and benefits:

Salary range: 27,000 -- 30,000
